Pay Range: $47,949.00 - $67,727.00 per year

The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.
Establishes working relationship with children who are developmentally delayed and their families,
Coordinates the scheduling of assessments in accordance with guidelines established by Early Intervention DOH,
Assessment of children s service needs in the following areas social emotional, developmental, familial medical, cultural, economic and educational,
Development and review of the Individualized Family Services Plan IFSP for all eligible Part C children,
Ensures all appropriate notification is provided,
Facilitates the coordination of services for Part C eligible children and families through linkage to service providers and EI programs,
Promoting and facilitating communication among service providers working with the child and family,
Advocating for appropriate services,
Supportive counseling and crisis intervention for Part C eligible children and families,
Assist families with transition planning to preschool, special education or other services considered appropriate at age three,
Ability to manage electronic files including data entry into EIMS DOH database,
Facilitate functioning of the Part C Unit in the absence of the Unit Coordinator,
Participates in the development, evaluation and revision of the Units policies and procedures,
