JOB SUMMARY
The Sepsis Program Coordinator’s primary focus is on the maintenance and continuous quality improvement of UVA Community Health’s (UVA CH) sepsis practices. The Sepsis Coordinator collaborates with the health team in all aspects of patient care. Provides formal and informal program education for provides and team members. Collaborates with nursing staff across the system to facilitate patient care and with health management regarding appropriate discharge options. Collects and manages clinical, financial, quality, and disease specific related care data.
Program Management: Serves as a liaison and resource to frontline team members, leaders, and providers and evaluates the Sepsis care practices to ensure alignment with evidence-based standards of care. Acts as a change agent in areas of clinical practice to achieve program goals and objectives. Organizes and facilitates Sepsis meetings across UVA CH. Evaluates interdisciplinary care processes to ensure consistency in the standards of care and practice throughout the care continuum. Assists with the development and revision of protocols, standards, and educational materials to ensure that they are current and meet the accepted national standards of care practice. Participates in, and provides coaching and education for, Sepsis care process. Coordinates UVA CH Sepsis education programs and community Sepsis events. Plans and implements Sepsis education tools, presentations, and screenings to increase Sepsis/Infection awareness in the community. Plans and implements activities for Sepsis Awareness Month.
Program Representative: Serves as a liaison to staff, physicians, Emergency Medical Services, and Community Services regionally. Leads and evaluates the Sepsis process and serves as a resource to nursing, health management specialist, and physicians. Acts as a change agent in areas of clinical practice to achieve program goals and objectives.
Data Management: Utilizes/compiles databases specific to the patient population and relevant to resource utilization, practice patterns, patient processes and outcomes regionally. Compiles data to establish profile for state and national comparison and goal setting to improve clinical outcomes and to meet certification and regulatory requirements. Supports and fosters evidence-based practice. Assess educational needs of the inpatient and outpatient Sepsis population and develops educational materials/tools to facilitate learning and increase Sepsis awareness. Manages data from Sepsis logs and provides just in time feedback to care teams.
Patient Care: Works with team members to develop or improve processes to assure a remarkable patient experience.

QUALIFICATIONS
Education: 4 Year/Bachelor’s Degree required. Master’s Degree preferred.
Experience: Minimum of 3 years of recent/relevant clinical experience in acute care setting required. Refer to the Life Support Training Policy for additional details.
Licensure: Current RN licensure in applicable state required.
Additional Skills/Requirements Required: Strong grasp of data analytics including the ability to abstract, analyze data, and utilize data to drive performance improvement projects.
Additional Skills/Requirements Preferred: ED and/or Critical care experience strongly preferred.
