Coordinator, Palliative Care RN, Virginia Hospital Center
Job Summary: In general, the Palliative Care Nurse Coordinator is responsible for screening palliative care referrals/ evaluating the appropriateness of palliative care referrals, gathering patient and family data in preparation for the palliative care consult by the palliative care team, coordinating palliative care consults, assessing member palliative are needs and monitoring the quality of outcomes. Essential Responsibilities: + Maintain the integrity of palliative care referrals: Screen, assess and prioritize palliative care consult referrals; Schedule and plan palliative care consults for the palliative care team while being sensitive to time constraints and resource. Position is RN member of onsite Kaiser Permanente Palliative Care Team at Virginia Hospital Center. Basic Qualifications: Experience + Minimum of two (2) years of clinical nursing practice experience required. Education + Successful completion of palliative care training within 12 months of hire required, including; pain and symptom management, nutrition and hydration, psychosocial and spiritual care and hospice. License, Certification, Registration + This job requires credentials from multiple states. Credentials from the primary work state are required at hire. Additional Credentials from the secondary work state(s) are required post hire. + Registered Nurse License (Virginia) within 6 months of hire AND Registered Nurse License (Maryland) within 6 months of hire OR + Compact License: Registered Nurse within 6 months of hire + Registered Nurse License (District of Columbia) within 6 months of hire + Basic Life Support from American Heart Association + Certified Hospice and Palliative Nurse within 18 months of hire Additional Requirements: + Ability to effectively communicate. + Ability to conform to established policies and procedures + Ability to analytically problem solve and make decisions + Ability to tolerate and cope with ambiguity + Ability to work as a team member + Proficiency in the use of applicable computer software. + Demonstrated ability to effectively and clearly present information through the written word, to influence or persuade others through oral presentation in positive or negative circumstances, and to listen well. + Demonstrated ability to write clearly, legibly, and effectively, to present ideas and document activities, to read and interpret written information. Preferred Qualifications: + Two years of experience in hospice and palliative care setting preferred. + Experience in acute care, intensive care, emergency, skilled nursing facility, long-term care or managed care settings preferred. + B.S.N. preferred.
