Responsible for intake and orientation for new CDC referrals and determining eligibility; carry a caseload of consumers who are enrolled in the CDC service model. This position will also be responsible for assisting with marketing the CDC service model in and around the Greater Lynn area. This includes providing presentations to small groups of people, as well as individual presentations, and other activities specific to growing the model as needed. Position Responsibilities: Perform intake and orientation on new CDC referrals. Instruct the consumer in the rules, policies, and regulations and to describe the role of the Aging Services Access Point and Fiscal Intermediary. Perform ongoing service agreement reviews on new consumer/surrogate and provide additional skills training as needed. Maintain consumer's date, care plans, and progress notes in SAMS, Community Care Platform (CCP) and case file. Qualifications + Bachelor's degree in Social Work, Human Services or other related field preferred + Prior experience in Human Services, preferably with Elders + Knowledge of Community Resources preferred + Possess the knowledge and skills necessary to work successfully as a member of an interdisciplinary team to provide or arrange for community long-term care + Ability to make proper judgments, demonstrated leadership qualities, good written and verbal skills, and ability to maintain effective interpersonal relationships with staff members, consumers and partners + Proficient in the use of computer systems, including but not limited to, Windows based software application including but not limited to Word, Excel, PPT and Outlook + Ability to process information accurately + Ability to work both independently and as part of a team + Professional demeanor and ability to handle confidential materials with discretion + Must have reliable transportation and valid driver's license for community outreach and home visits + Bilingual speaking preferred Hours: Full Time- 30 hours Monday through Friday *GLSS is an affirmative action and equal opportunity employer.
