What We Offer The Novant Health Hallmark Care team is excited to add another experienced Primary Care Physician to their team. This is an outpatient only, concierge-style family practice which offers its patients extensive access to their physicians, longer appointment times, and personalized care plans that help patients optimize all of the pillars of good health and wellness. In addition to providing general primary care and management of chronic conditions, our doctors practice proactively—incorporating advanced preventive and diagnostic testing to identify health risks early and guide personalized longitudinal care. Compared to a traditional primary care practice, doctors at Novant Health Hallmark care benefit from smaller patient panels and extended encounter times, usually seeing about 8 to 10 patients per day. Our providers are encouraged to practice medicine “as it was meant to be”- engaged, relationship-based, and centered on true patient advocacy. Our physicians are supported in taking the time to think critically, to partner with patients, and to remain actively involved in all aspects of their care. Novant Health Hallmark Care is open Monday through Friday from 8:00 AM to 5 PM. Patients are seen in person and via video and televisits. Physicians participate in a rotating, phone – only call schedule.
