The position will report directly to the executive Director, Medicaid, and Health Services Branch. The position will be responsible for the design, development, and implementation of the complex care management initiative with support from MHSB and Division leadership. The member population will include significantly complex medical conditions and/or social-economic or mental health co-morbidities.
