participate in their own care and recoveryProgram SummaryNatchaug Hospitals Journey House program is a bed specialized residential treatment facility on the Mansfield campus for adolescent girls ages to who are involved in the court system Referred girls have identified needs for behavioral health services and may have a history of one or more of the following circumstances significant trauma mental illness victim of Domestic Minor Sex Trafficking a history of substance abuse and assaultive andor self injurious behaviorsJob SummaryCommunity and Vocational Coordinator works directly with residents and their families in the community and within the program while in placement at Journey House They will be responsible for assessing and building vocational skills of the residents and partnering with the families to follow up on therapeutic goals at discharge They will be accountable to continue to work on discharge goals and vocational skill development through days post dischargeKey responsibilitiesWorks with the clinical team Clinicians Mental Health Workers Nurses etc in the development and implementation of treatment plans both during placement at the Journey House and after discharge Conducts life skill assessments and develops a plan to improve those skills ARISE modelTransports residents and families during placement at Journey House and during the
