Key Responsibilities:
· Deliver community-based mental health services including assessment, diagnosis, treatment planning, and implementation
· Collaborate with clients and families to create customized treatment plans aligned with client needs and goals
· Provide behavioral interventions using evidence-based techniques such as CBT, DBT, Motivational Interviewing, and Trauma-Informed Care
· Coordinate care with schools, physicians, psychiatrists, and community organizations
· Facilitate and lead PSR group sessions in alignment with client treatment goals
· Maintain clear and timely documentation of services, interactions, and client progress
· Develop strong professional relationships with stakeholders, including teachers, parents, counselors, and external providers
· Participate in training and professional development activities to enhance skills and knowledge
· Support organization’s quality improvement efforts through data collection and reporting
· Perform other duties as assigned by supervisor

Work experience in a related human services field may substitute for educational requirements. *Related fields may include direct support such as case management, residential support, developmentally disabled support, vocational rehabilitation, and more. Four years of qualifying experience minimum or a combination of education and experience equaling four years required.

Qualified Addictions Professional (QAP) - CADC, CRADC, CRAADC, CCJP, RADC-P, RADC, CCDP, CCDP-D
Additional Qualifications:
· Must be at least 21 years of age to operate a company-owned vehicle
· Must be at least 23 years of age to operate a 15-passenger company-owned van
· Must have reliable transportation to transport clients in personal vehicle
· Current driver’s license, acceptable driving record, and current auto insurance required
· Strong ability to problem solve, self-motivate, and communicate effectively
· Ability to manage multiple priorities while maintaining a high standard of client care
· Must obtain and complete trainings that support DMH standards and professional development
Physical Requirements:
ADA Consideration - Sedentary work: Exerting up to 10 pounds of force occasionally (up to 1/3 of the time) and/or a negligible amount of force frequently (1/3 to 2/3 of the time) to lift, carry, push, or pull, or otherwise move objects, including the human body. Repetitive movements of hands, fingers, and arms for typing and/or writing during work shift.
Sedentary work involves sitting most of the time but may include walking or standing for brief periods. Jobs are considered sedentary if walking and standing are required only occasionally, and all other sedentary criteria are met.
