As a Community Support Specialist, you'll provide comprehensive mental health services, including assessment, diagnosis, treatment planning, and intervention implementation. Through a strengths-based approach that emphasizes empowerment, skill-building, and resilience, you'll foster growth, stability, and resilience in your clients. By connecting them with local resources and building a robust support network, you'll be instrumental in their journey towards independence and overall well-being.
Key Responsibilities:
Provide community-based mental health services to clients through assessment, diagnosis, treatment planning, and intervention implementation.
Collaborate with clients, families, and stakeholders to develop and implement personalized treatment plans tailored to specific needs and goals.
Utilize evidence-based practices and strengths-based approaches to empower clients and build resilience.
Coordinate care with other service providers, including primary care physicians, psychiatrists, schools, and community organizations.
Maintain accurate and timely records of client interactions, treatments, and progress.
Participate in staff development activities to enhance knowledge and skills related to youth mental health services.
Contribute to the organization's quality improvement initiatives through data collection, analysis, and reporting.
Foster collaborative relationships with team members, stakeholders, and professionals involved in the client's care.
Facilitate PSR group sessions aligned with client treatment plan goals.
Education and/or Experience Qualifications:
Bachelor's degree in one of the following fields of study:
Psychology, Social Work, Sociology, Education, Criminal Justice, Family Studies, Counseling, Recreational Therapy, Human Services, Human Development, Child Development, Gerontology, Behavioral Science, Rehabilitation Counseling OR...

Additional Qualifications:
Must be at least twenty-one (21) years of age to operate a company-owned vehicle.
Must be at least twenty-three (23) years of age to operate a company-owned fifteen (15) passenger van.
Knowledge of case management, rehabilitation methods, and principles related to mental health and/or substance use disorders.
Ability to problem-solve, communicate effectively, and produce clear and concise reports.
Ability to multi-task, coordinate services from community resources, and maintain effective communication with staff.
Ability to establish effective relationships with clients and their families.
Ability to follow directions and accept supervision.
Valid driver's license in state of residency with an acceptable driving record.
Reliable means of transportation and proof of valid and current auto insurance.
