As a Community Support Specialist, you'll help individuals adapt to living in their communities by creating personalized plans that address their specific needs. You'll use evidence-based techniques and collaborate with other professionals to provide comprehensive mental health services.

Key Responsibilities:· Provide community-based mental health services to children, adolescents, and/or adults, including assessment, diagnosis, treatment planning, and intervention implementation
· Collaborate with clients, families, and other stakeholders to develop and implement individualized treatment plans
· Utilize a strengths-based approach that emphasizes empowerment, skill-building, and resilience
· Provide behavioral interventions, including motivational interviewing and cognitive processing
· Coordinate care with other service providers
· Develop and maintain relationships with key stakeholders, including parents, teachers, and counselors
· Maintain accurate and timely records of client interactions and progress
· Participate in staff development activities and quality improvement initiatives

· Bachelor's degree in one of the following fields of study: Psychology, Social Work, Sociology, Education, Criminal Justice, Family Studies, Counseling, Recreational Therapy, Human Services, Human Development, Child Development, Gerontology, Behavioral Science, Rehabilitation CounselingOR...· Associate of Applied Science in Behavioral Health Support degree from an approved institutionOR....· Work experience in a related human services field may substitute for educational requirements. *Related fields may include direct support such as case management, residential support, developmentally disabled support, vocational rehabilitation, and more. Four years of qualifying experience minimum or a combination of education and experience equaling four years required.

Additional Qualifications:· Knowledge of case management and rehabilitation methods, principles and techniques as they relate to mental health and/or substance use disorders
· Ability to problem solve and communicate ideas clearly and effectively
· Ability to be self-motivated to provide high-level performance while meeting the complex needs of the clients and their families
· Ability to multi-task and coordinate services from a variety of community resources
· Ability to establish effective relationships with youth and their families via telephone and personal contacts
· Ability to follow direction and accept supervision
· Successful completion of background check including criminal record, driving record, abuse/neglect and fingerprint check
· Current driver's license, acceptable driving record and current auto insurance

Physical Requirements:· ADA Consideration - Sedentary work: Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently to lift, carry, push, or pull, or otherwise move objects, including the human body
· Repetitive movements of hands, fingers, and arms for typing and/or writing during work shift
· Sedentary work involves sitting most of the time but may involve walking or standing for brief periods of time
