The Community Resource Navigator is a healthcare worker who is a trusted member of and/or has an unusually close understanding of the community served. This role supports patient care in accordance with established methods, techniques, and standards of The Corvallis Clinic. This position will work and bill under the supervision of a licensed mental health provider, and report to the Pediatric Practice Manager.

Schedule: Monday through Friday 8 am to 5 pm.PST

Location: 3680 NW Samaritan Drive, Second Floor, Corvallis, OR 97330

Primary Responsibilities:

Will participate and maintain a culture within The Corvallis Clinic consistent with the content outlined in the Service and Behavioral Standards Caregiver document. To this end, employee will be expected to read, have familiarity with, and embrace the principles contained withinCare Coordination:Coordinates with involved systems of careAssists with referralsContributes to team care plans and planningAssists with transitions between providers and phases of careConnects people to community and/or social service resourcesOutreach and Direct Service:Provides follow-up with patients and their families, if appropriateDeliver screenings and health education related to prevention and healthy behaviors:Assists with mental health media outreachCoaching and Social Support:Provides resources for social support and build social networksMotivates and encourages people to obtain care and servicesAdvocacy, Organizing, and Cultural Mediation:Advocates for the needs and perspectives of patientsConducts two-way education about community resourcesNavigators will perform the following functions:Works with patients and families to teach them to interact effectively with the healthcare systemSupports the assessment, design, development, implementation, and evaluation of the Care Coordination Navigator Program:Assists providers in creating and implementing new mental health initiatives for patients and trauma-informed clinical practicesPerforms other duties as assigned

Required Qualifications:

High School Diploma/GED (or higher)One (1) of the following required:2+ years of experience in a behavioral health setting1+ years of experience in a healthcare settingCertificated Health Worker (CHW) with the Oregon Health AuthorityIntermediate level of computer proficiency (including Microsoft Outlook, Teams) and be able to use multiple web applications

Preferred Qualifications:

Knowledge of Medicaid/Medicare populationKnowledge of Outpatient, ambulatory care and/or community healthcareKnowledge of Care coordinationKnowledge of A primary care related settingKnowledge of Chronic conditionsKnowledge of Electronic medical information systems

Soft Skills:

Strong communication and customer service skills both in person and via phone Ability to work independently and maintain good judgment and accountability Demonstrated ability to work well with othersStrong organizational and time management skills Ability to multi-task and prioritize tasks to meet all deadlines Ability to work well under pressure in a fast-paced environment Excellent verbal and written communication skills; ability to speak clearly and concisely, conveying information in a manner that others can understand, as well as ability to understand and interpret information from others 

 

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $16.00 to $27.69 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
