General Summary
The Community Health Worker (CHW) functions as a critical member of the Women's and Children's Care Management team within the Department of Population Health. This role supports pediatric and obstetric patients across West York, Adams, and Franklin Counties, working directly with families to reduce barriers to care, improve health outcomes, and promote health equity.
The CHW serves as a trusted liaison between patients, providers, and community resources, helping families navigate complex health and social systems. Through proactive outreach, in-person engagement, and interdisciplinary collaboration, the CHW supports patients experiencing social determinants of health (SDOH) challenges and assists in stabilizing high-risk situations.
