Job SummaryThe Community Health Worker 2 provides cultural mediation between health care, social services, and the community. This position helps the community access needed services. It builds personal, community, and system capacity by increasing health knowledge and self-support. Activities include outreach, patient navigation, follow-up, community health education, informal counseling, social help, advocacy, and participation in clinical research.
Essential Functions of the RoleConducts needs assessments and develops patient plans. Obtains basic vital signs, measures height and weight, and conducts point of care testing.Navigates patients to interventions that improve access to health care and health status. Provides referral and linkage to follow-up services in the community and the Organization.Conducts culturally appropriate skills building self-management education sessions on different topics for groups and people.Teaches basic concepts of health promotion, disease prevention and self-management.Provides informal leadership to other community health workers including peer observation. Participates in mentorship program, as assigned.Promotes culturally competent care by educating staff about beliefs and practices relevant to specific populations.Documents plans, goals, education and follow-ups in a timely manner, utilizing concurred upon documentation standards.Represents the program on councils and committees at various levels within the organization. QUALIFICATIONS
EDUCATION - H.S. Diploma/GED EquivalentEXPERIENCE - 2 Years of ExperienceCERTIFICATION/LICENSE/REGISTRATION -Community Health Worker (CHW): CHW license by the State of Texas.
