Job SummaryThe Community Health Worker 1 provides health services in needs assessments, referral services, client education, documentation, and diversity education.
Essential Functions of the RoleAssists clients in accessing services and increasing health knowledge and self-sustainment. Learns to use tools and services like outreach, patient navigation, and community health education. Assists with referrals and linkage to follow-up services in the community and the Organization.Assists with needs assessments and the development of patient plans.Conducts culturally appropriate skills-building self-management education sessions on various topics for groups and individuals. Builds knowledge of the community's ethnicity, language, socio-economic status, and life experiences.Learns and assists in teaching basic concepts of health promotion, disease prevention, and self-management.Obtains basic vital signs, measures height and weight, conducts point of care testing.Administers evaluation tools to assess program effectiveness.Assists in documenting plans, goals, education and follow-up in a timely manner, utilizing concurred upon documentation standards.Key Success FactorsKnowledge of the community to be served.Excellent communication skills, both verbally and in writing.Excellent interpersonal skills that encourage relationship building with a diverse group.Requires the capacity building and advocacy skills.Teaching skills preferred.Good organizational skills.Skilled in the use of computers and related software applications.Ability to administer evaluation tools to assess program effectiveness.Ability to conduct needs assessments, develop patient plans, conduct and navigate patient to appropriate interventions.Belonging StatementWe believe that all people should feel welcomed, valued and supported.
QUALIFICATIONS
EDUCATION - H.S. Diploma/GED EquivalentEXPERIENCE - Less than 1 Year of ExperienceCERTIFICATION/LICENSE/REGISTRATION -Community Health Worker (CHW): CHW license by the State of Texas.
