The CHN serves as an advocate for patients, providing support and education so they are better able to engage in their care. This position offers a hybrid work opportunity, with some travel between hospitals for onsite responsibilities coupled with working remotely. **The value you'll bring to the team:** + Works closely with the healthcare team, resource providers and insurance companies. + Connects with patients prior to discharge to better understand contributing factors/barriers to their dependence to help identify social and/or resource needs. + Educates patients and families re: available community resources, programs and social services for alcohol and/or drug dependence. + Assists patients with navigating insurance applications, verification, and authorization; counseling services and/or other available resource providers tailored to their social needs; and helps in scheduling appointments. + Conducts post-discharge phone calls to provide overall support and encouragement, obtains updates from patients for their perspective on their recovery progress, and collaborates with resource providers when additional assistance needs are identified by patients.
