Community Health Worker

Responsibilities

The Community Health Workers (CHWs) role will to be to conduct outreach to recruit faith-based community churches to act as community hubs for chronic disease prevention information dissemination.

Specific goals include:

Outreach and building relationships with faith-based community churches

Identifying a champion at each site and training each champion in the Diabetes Empowerment Education Program (DEEP) curriculum

Empower identified champions to lead DEEP workshops in their communities

Program goal: increase awareness, prevention, and education to African American, Hispanic, Native American, Asian, and Pacific Islander populations who are at risk of life-threatening kidney disease

**This position pays $23.00**
