The Integrated Care Management Program (iCMP) is designed to reduce complication by supporting patients on their health care journey. Our iCMP nurses, social workers, and community health workers meet with patients and assess their health and social needs. Together, they identify barriers to good health and create a plan to address them. The team works with our patients and their providers to create a personal care plan to support the patient in achieving optimal health and well-being.
Job Summary
The Opportunity• Works closely with individuals and communities to promote and maintain good health.
• Provides education, support, and outreach services to community members to help them achieve their health goals. Areas could include exercise, mental health, medication management, nutrition, health care system navigation, substance use and other health behaviors.
• Works under the direction of a healthcare professional to provide preventive care and health education to members of the community.
• Conducts community outreach and engage community members to understand their health needs.
• Provides individual and group health education and coaching to community members.
• Assists community members in accessing and navigating the healthcare system, including providing referrals to appropriate health services.
• Provides basic health screenings and assessments to identify health risks.
