This role works closely with partners across the organization, system, and/or community including government agencies, faith-based institutions, and non-profit organizations to ensure the community health programs run smoothly. Essential Job Functions + Assists with planning and implementationof community health program(s) in support of advancing Community Health goals. + Participates in outreach efforts, which may include engaging with local organizations, community members, and care providers to facilitate collaboration and program delivery by promoting program participation andraising awareness of health services. + Coordinates operations and support related to planning, communication/promotions, logistics, and documentation of community health initiatives to reach diverse populations. + May collect, track, and report program data to leadershipto support continuous improvement and compliance. + Provides input into policies and programs that improve the health of the community served including new interventions and improved delivery of health services. _This document is not an exhaustive list of all responsibilities, skills, duties, requirements, or working conditions associated with the job.
