Job Summary and Responsibilities

As our Community Health Worker, you will be a vital link between individuals and essential community resources, actively working to reduce health disparities and improve access to support services.

Every day, you will proactively engage with clients, assessing their needs and identifying pathways to care and support. You'll connect individuals to relevant health, social, educational, and employment resources, providing guidance and practical assistance to overcome logistical barriers.

To be successful in this role, you will possess a profound understanding of diverse community needs, exceptional problem-solving abilities, empathy, and resilience. Your dedication to empowering clients and removing barriers to care will be paramount.

Participate as a full member of the CHP Ambulatory Social Work & Community Health team, including attending team meetings and care conferences as appropriate as well as attend PCH required trainings and monthly meetingsVerify eligibility, complete consent form, and all documentation (Release of Information, Demographic Form, Visit Form, Progress Form and Standard Pathways) and tools as needed for enrolling the population being served;Educate enrolled participants on topics relevant to their situation using the PCHI ® Model Learning Modules;Work with enrolled participants and families on needs that may or will affect the participant’s health, and connect them to community-based resources and services to remove barriers to care, including transportation issues, housing, financial concerns, etc.Act as a peer support and advocate for participants as they navigate the medical and social service system.Identify and link participants and families to community resources and services, assisting with coordination as necessary
Job Requirements

Required: High School Diploma

Preferred: Community Health experience/education, Social Worker BSW, or similar experience/education
