Position Summary
The Community Clinical Liaison is responsible for developing and maintaining positive relationships with referral sources to increase access to Home Health, Hospice, Home Infusion, PACE, Transitional Care Unit, and other services within assigned accounts. This role collaborates with discharge planners, facility staff, physicians, and patients to assess appropriateness for services, coordinate transitions of care, and ensure documentation meets agency and regulatory standards. The Liaison also provides education and consultation on home-based care options, end-of-life care, pain and symptom management, and related topics for patients, families, physicians, and community partners.
The pay range for this position is: 35.06-54.43 hourly.
Position Specific Responsibilities
• Coordinate discharge planning with hospitals, clinics, physicians, and community providers to ensure seamless transitions of care.
• Provide education and consultation to hospital staff, patients, families, and physicians about home health, hospice, and infusion services.
• Serve as liaison between patients, families, payors, physicians, and referral sources, ensuring accurate orders and referral information are provided to intake.
• Consult with attending physicians, Hospice Medical Directors, and other care team members to support patient and family needs.
• Facilitate admissions to Home Health, Hospice, and Infusion Pharmacy services, ensuring consents and documentation are complete and compliant.
• Support expansion and implementation of the General Inpatient (GIP) Hospice Program.
• Discharge GIP patients to appropriate care settings (home, SNF, or ALF) in coordination with the hospice team.
• Maintain and build referral base relationships by serving as a clinical resource and educator for facility staff, physicians, and community partners.
• Appropriately document activities in Providence’s electronic medical system and track referral outcomes.
• Participate in strategic initiatives to support referral growth and business development within assigned accounts.
• Provide education to staff and physicians on end-of-life care, hospice, and GIP services.
• Follow up on referrals to ensure all services and products are arranged.
• Perform other duties as assigned.

Physical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential.
