in coordination with interdisciplinary teams to organize prepare and deliver evidence based substance use and addiction treatment services Maintains ethical principles and professional standards of practice in adherence to the disciplinersquos professional code of ethicsCollaborates with the interdisciplinary team to conduct a comprehensive bio psychosocial assessment develops and implements treatmentdischarge plan monitors progress of interventions and revises plan to meet patient and family needs and coordinates consultsreferrals to remove barriers and promote the individualized care plan detox andor substance use rehabProvides education to patientfamily with respect to meeting established goals including but not limited to disease prevention and progression advocacy for benefits and maintenance of treatment regimenProvides direct patient care that may include case management care coordination interdisciplinary collaboration for care transition needs crisis interventionsafety planning counseling and emotional support to patients and families coping with illness trauma referral and discharge planningMaintains timely patient centered documentation that reflects the patient and client systemsrsquo pertinent information for assessment interventions and outcomes in accordance with regulatory licensing requirements and third party agenciesParticipates in administrative and
