Job Overview:
The Clinician screens new patients and provides concrete social services to patients for financial, transportation, housing, legal, AA/NA meetings and day care services. The Clinician also facilitates manualized and psycho-educational groups in the IOP and OP programs.

Attends all clinical team meetings and assists with tracking treatment plan/treatment plan due datesAttends clinical conferences when requestedFacilitates transfer of patient or patient information from one unit to another according to policy and proceduresProvides on call services for new referrals and admissionsRefers to PES for afterhours crisis issuesProvides social service assessments as requestedFollows up on these referrals as necessaryProvides case management services for Co-occurring patientsProvides outreach for co-occurring patients who have been absent from programProvides transportation services to programming, social services and other concrete services required by the clientFacilitates psycho-educational groupsBenefits and Perks:
