Clinical Transformation Lead
**General Summary** Drives measurable quadruple aim improvements across the care continuum in support of WellSpan's population health and value-based care strategies. The role is responsible for leading complex, patient-centered clinical improvement initiatives that improve care quality and eliminate low value medical utilization, coordinating cross-functional stakeholders across service lines and independent practices, and applying data analytics, process improvement methods, and emerging AI technologies to implement scalable, evidence-based clinical practices. This position also fosters a culture of continuous improvement and population health accountability across the WellSpan enterprise. Expected results include sustained performance improvement on key population health metrics, reduced provider burden, and stronger organizational capabilities to deliver high-quality, affordable, and reliable care. **Duties and Responsibilities** **Remote Work Capable** **Essential Functions:** + Co-owns and drives a portfolio of total costs of care initiatives across employed and independent practices and clinical departments to eliminate medical spending that does not contribute to optimal health outcomes. + Builds and maintains effective relationships with clinical, operational, and executive leaders to support a continuous cycle of opportunity identification, prioritization, project design, execution, and systematization. + Serves as a subject matter expert in the Quadruple Aim, value-based care, and population health clinical improvement models, translating these concepts into actionable strategies for clinical teams. + Coaches frontline teams and service line leaders on change management, performance improvement methodologies, and data-driven decision-making. + Leads cross-functional teams to execute complex initiatives, drawing on specific expertise of finance, performance improvement, informatics, and other WellSpan enterprise functions as needed. + Applies working knowledge of finance, electronic health records, data systems, and emerging technologies (including AI and machine learning) to support project design, implementation, and scalability. + Partners closely with business analysts to leverage data insights, develop business use cases, design interventions, and quantify project outcomes including savings and quality improvements. + Monitors project performance and key population health metrics, supports low performing practices, and makes recommendations when initiatives are not meeting targets. + Develops and delivers regular reports and executive summaries to service lines and population health leadership on initiative progress, outcomes, and risks. + Monitors internal benchmarks and industry trends and fosters a culture of continuous improvement, shared learning, and rapid-cycle testing to advance total costs of care transformation. **Common Expectations:** + Demonstrates strong emotional intelligence, adapting communication and facilitation styles to build productive relationships and lead diverse stakeholders toward shared outcomes. + Applies advanced problem-solving and analytical skills, using clinical and operational data to inform decisions, prioritize opportunities, and drive measurable change while reducing provider burden. + Exercises sound judgment, adaptability, and disciplined project and change management practices in management of complex initiatives and competing priorities. **Qualifications** **Minimum Education:** + Bachelors Degree Required + Masters Degree In related field. Preferred **Work Experience:** + 3 years Experience in leading clinical or operational improvement initiatives in health care settings. Required and + 3 years Experience in clinical settings or clinical operations with preference for clinical licensure. Required and + 3 years Experience in change management, population health, value-based care, or total costs of care transformation.
