What We Offer Schedule: Monday - Friday 8:00 AM - 5:00 PM; Full-Time Department: Care Navigation - supporting heart failure patients Location: Presbyterian Medical Center - Charlotte, NC; Presbyterian is our 661 bed level 2 trauma center centrally located just outside of uptown Charlotte. This role is ideal for an experienced RN looking to serve as the primary liaison between the patient and his/her medical team and be a patient advocate. + A dynamic full-time opportunity supporting cardiac patients + Work/life balance with no nights, weekends, holidays, or call + Access to continuing education and leadership support + Inclusive culture grounded in teamwork, trust, and shared purpose What You'll Do + Support heart failure patients on any unit at PMC + Find resources for patients including food, medicine, and clinics + Work to discover what is causing the patient to return to the hospital + Review medication and diet adherance issues with the patient + Provide some educaiton on the patient's diagnosis + Partner with case management and palliative care teams + Work towards goal of reducing readmission and ER visits What We're Looking For + Education: 4 Year / Bachelors Degree, required. + Experience: Three to five years of nursing experience, required. Two years of experience in care of targeted patient population; experience in a broad range of clinical situations or settings, preferred. + Licensure/Certification: RN, required. + Additional Skills/Requirements (required): Ability to successfully complete generic and department-specific skills validation and competency testing.
