Note: Benefits may vary based on position type and/or level **LOCATION** This position will support BSWH Waxahachie Medical Center and Health Texas Provider Network (HTPN) Clinics. The primary home office for this position will be in the Waxahachie hospital with travel to the HTPN Clinics when expected. **Job Summary** The Clinical Risk Manager (CRM) directs and coordinates clinical risk management for the hospital. The CRM integrates information from adverse events, near misses, RCAs, and clinical risk issues. With this information, the CRM reviews findings and works with Patient Safety and hospital managers. Together, they develop programs to improve a culture that supports patient safety and reduces risk exposure. **Essential Functions of the Role** + Reviews adverse event reports and obtains more detailed information by chart review and interviews as indicated. + When adverse events cause harm to patients, the CRM under the direction of the Corporate Director of Risk Management, investigates in detail to evaluate degree of patient harm. The CRM will participate in decision-making regarding the implementation of an early intervention. + Addresses day to day questions and problems involving patient care or concerns. Assists Patient Relations to address patient complaints or incidents and works to resolve issues before they develop into liability claims. + Receives information from other team members who are accountable for complaint and Ethics Hotline management about specific patient care processes that may create risk of patient harm and participates in the investigations as appropriate. + Assists Patient Relations to address grievances that involve patient care and responds to the patient in the time frame designated by CMS. + Is a trusted agent for clinical staff to report concerns about aspects of care that may generate risk to patients. + Participates in the effective disclosure of adverse events to patients and families by serving as a consultant and educator to those accountable to disclose this information. + Reports to BSWH department of Risk Management and facility senior clinical and executive managers a) the initial existence of the serious adverse events, concerns or risks and the plan for investigation; b) the results of the initial investigation; c) the plan for more detailed investigation (RCA or other) and d) the? lessons learned? from study of the event. + Assists the Manager of Insurance in managing claims related to property loss and visitor injury through early reporting and providing investigative information as appropriate. + Provide clinical Risk Analysis that is based upon priorities and specific hospital performance. + Analyzes trends of adverse events as reported by an electronic data source and presents results to Hospital executive leaders. + Investigates and prepares for the root cause analysis of sentinel events to include the development of a timeline, determining all individuals involved, scheduling and participating in the meeting, analyze all information received. + Sentinel Event documents are completed and reported to corporate risk management on a monthly basis. + Collaborates with Patient Safety and other disciplines to review adverse event reports for the purpose of recognizing trends and ensuring process improvement activities have been completed as deemed necessary. + Regularly reports to executive and medical staff managers related to CRM trending data and 'lessons learned'. + Develops and presents education programs for clinical staff to enhance their awareness and understanding of their role in reducing patient safety events for the goal of avoiding patient injury and reduce liability exposure. **Key Success Factors** + Bachelor's Degree in a healthcare field and four years of clinical experience. Or, an Associate's Degree in a healthcare field and six years of clinical experience.
