Your recruiter will provide the most up-to-date details during the hiring process._ **Responsibilities** Function as a liaison between internal/external referral sources and inpatient rehabilitation operations. + Establish relationships and aid with communication to case managers, physicians, payors, patients, families and referral sources in order to generate referrals and drive patient admission volume in the inpatient rehab. + Ensure appropriate patient selection through clinical chart assessments during on/off-site clinicalevaluation of patients in collaboration with the physician. + Complete in-service and educational opportunities for physicians, case managers, interdisciplinary members and other professionals. Serves as an educational resource to patients, family members and caregivers. + Participate in the development and implementation of business plans for the rehab unit and contributes to the development and maintenance of referral source promotional information. **Requirements** Licensure / Certification / Registration: + One or more of the following required: + Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date. + Rehab Therapist specializing in Occupational Therapy (OTR) credentialed from the Wisconsin Medical Examining Board obtained prior to hire date or job transfer date. + Rehab Therapist specializing in Physical Therapy (RPT) credentialed from the Wisconsin Medical Examining Board obtained prior to hire date or job transfer date. + Speech-Language Pathologist credentialed from the Wisconsin Hearing and Speech Examining Board obtained prior to hire date or job transfer date. + BLS Provider preferred. American Heart Association or American Red Cross accepted. + Drivers License preferred. Education: + High School diploma equivalency with 2 years of cumulative experience OR Associate'sdegree/Technical degree OR 4 years of applicable cumulative job specific experience required. **Additional Preferences** **Previous experience in long term care/inpatient rehab or acute rehab setting. Ability to be self directed, prioritize patients to be seen, comfortable with electronic health record navigation. Excellent verbal and written communication.
