**Job Description**
MSS seeks an **Onsite Health Nurse Practitioner** to provide primary care, well-woman care, and clinical oversight to an onsite employer health center. This position will report to the Medical Director.
The Onsite Health Nurse Practitioner will provide an excellent patient experience for all patients of the health center. The candidate will also collaborate with our business and operational teams on developing innovative presentations and clinical campaigns in the workplace.
**Qualifications**
Education and Experience:
Licensure: New York State License with current registration as a registered nurse.
State Certification: Certified by the New York State Department of Education as a Nurse Practitioner as follows:
1. Is a graduate of New York State registered Nurse Practitioner Program or equivalent OR is certified as a Nurse Practitioner by a national association via examination
2.Has completed a pharmacy component of not less than the required semester hours or the equivalent AND has been instructed in New York State and federal laws relating to prescriptions and recordkeeping (3-hour course).
3. Has a collaborative practice agreement with a physician and designated protocols, both filed with the New York State Department of Education.
+ At least three years of clinical experience as a Nurse Practitioner in an ambulatory care, Primary care treating adults, occupational health, family practice or emergency department setting. Additional Skills and Qualities Demonstrated problem-solving and workflow management skills. Strong computer skills with experience with electronic health records and some familiarity with presentation, spreadsheet, and word processing software. **Compensation Statement**
The Mount Sinai Health System (MSHS) provides salary ranges that comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $115,494.00 - $192,328.00 Annually. Actual salaries depend on a variety of factors, including experience, education, and operational need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
Non-Bargaining Unit, J04 - MSS Health Center Admin - ISM, Icahn School of Medicine
**Responsibilities**
1. Delivers primary care within the practice, through both traditional face-to-face visits as well as non-in person visit based interactions (e.g. virtual visits, phone, digital messaging).
2. Assists patients to understand diagnostic tests, treatment options, costs, and their health benefits.
3. Initiates referrals to specialty services and ensures bi-directional communication with specialists to promote continuity of care.
4. Works collaboratively with other staff to promote team-based care. Develops and administers wellness offerings
5. Participates in the development of wellness and health promotional offerings, including educational events, nutrition initiatives, health fairs, and campaigns. • Supports the planning, operation, and evaluation of wellness activities. Supports clinical operation and improvement activities
6. Assists the management team with developing, implementing, and managing clinical operations, policies and procedures. Responsibilities
7. Actively engages in practice innovation and improvement initiatives.
8. Monitors the clinical needs and workflows of the practice and ensures systems and programs are in place to provide exceptional, safe, and compliant care. Supports communication and marketing efforts
9. Participates in communicating and marketing the practice model to new patients as well as other stakeholders such as clients, patients, and third-party vendors.
10. Attends meetings with members of the Mount Sinai Solutions Team.
11. Provides continuous availability for medical and technical questions to the patients, patient care staff, management, and the client. Provides concierge-level service and navigation
12. Carries out other appropriate professional duties as may be necessary, including liaison with outside medical consultants, hospitalists, emergency room providers, etc. to ensure continuity of care, timely, appropriate care, and excellent patient experience.
13. Becomes knowledgeable on our clients’ employer-sponsored benefits to be able to answer questions around medical benefits, claims, care coordination and other complexities of the healthcare system. This will include explaining complicated medical and benefits terms in plain language.
14. Partners with the health center staff to develop seamless and timely navigation for patients to Mount Sinai health services.
