CLINICAL PROGRAM COORDINATOR

Summary:

Under the direction of the department manager and in partnership with Saint Alphonsus Health System colleagues, fulfills the delivery of Corporate Wellness offerings to Saint Alphonsus colleagues and area employers.

Requirements:

Must be licensed in the State of Idaho as a Registered Nurse as defined by the Idaho State Board of Nursing.At least one year of clinical experience required.Experience presenting health information to small groups preferred.

What you will do:

Provides wellness programs and services to Saint Alphonsus Health System colleagues and corporate clients by performing services including the following:

Collaborating with healthcare team to administer wellness projects and programming for employees and corporate clients.

Motivating individuals to make positive health changes and promote preventative care.

Working with team to develop and implement wellness programming and chronic disease prevention programs and classes.

Organizing and attending on-site corporate screenings and wellness events.

Reviewing lab results and providing patient education and feedback.

Communicating wellness services and providing outreach to at risk participants.

Providing ongoing chronic disease management.

Demonstrates compliance with hospital safety policies and procedures.
