Clinical Pharmacist Specialist
Full-Time I Second Shift
Hospital Pharmacy I Lexington, KY
The Department of Pharmacy Services of Baptist Health Lexington provides pharmaceutical services to patients across the continuum of care. These services include, but are not limited to verification of medication orders, drug distribution, sterile product preparation, hazardous drug preparation, and utilization of automated dispensing cabinets. In addition, decentralized pharmacists provide services such as management of pharmacy dosing consults, provision of drug information, therapeutic recommendations, and participation in daily multidisciplinary rounds.
Description for Pharmacist:
Works collaboratively with physicians and other members of the healthcare team to develop programs/guidelines that improve practice and patient outcomes.Serves as a drug information expert and resource regarding medication management.Provides comprehensive decentralized or pharmacy clinical services including, but not limited to, anticoagulation management/monitoring, pharmacokinetic dosing, renal dosing, and nutritional support services.Participates in antimicrobial stewardship and opioid stewardship.Provides medication counseling to patients.Works with inter-professional patient care teams to help patients with transitions to care, including but not limited to helping prepare patients for discharge.Serves as an active member of the inter-professional team on patient care units. Participates in unit-based huddles and activities if available. Knowledgeable in unit’s metrics, goals, and works with staff on unit to help achieve goals.Works collaboratively with physicians and other members of the healthcare team to develop programs/guidelines that improve practice and patient outcomes.Communicates with a high level of skill to interact with physicians, R.N.’s, patients, and other healthcare providers. Ability to solve problems.Provides verbal and written consultations concerning medication therapy on a daily basis.Provides education to patients, physicians, nurses, and other healthcare team members as requested.Participate in multidisciplinary rounds and provide evidence-based therapy recommendations as appropriate (when applicable).SpecialtyRequirements:
3 years of experience.Completion of PGY-1 or PGY-2 ASHP, accredited Pharmacy residency in area of practice.Board of Certification in Specialist within one year.Current Kentucky pharmacist license. BLS/ACLS within 6 months.Benefits:
