The Clinical Pharmacist - Nursing Unit Based is a decentralized role that ensures patients receive safe, effective, and appropriate medication therapy through a blend of clinical and operational responsibilities. Working closely with physicians, nurses, patients, and interdisciplinary care teams, this role rotates between clinical and operational functions, including patient profile review, medication management, consults, patient education, interdisciplinary rounding, and precepting, as well as order verification, renal dosing, and clinical support. Supporting a general inpatient population, this position plays a direct role in improving medication safety, optimizing therapy outcomes, and enhancing overall patient care within a collaborative, team-focused environment.
**A caregiver in this position will work day/evening rotations from 7:00am to 3:30pm or 8:00am to 4:30pm. Evening shifts are 3:00pm to 11:30pm once per week on rotation. Weekend rotations occur every 4th weekend and rotates between 1st and 2nd shift. Holiday requirements are 2 per year (1 major and 1 minor holiday). Occasional overnight coverage may occur (3-4 consecutive nights) approximately once per year for PTO backfill.**
A caregiver who excels in this role will:
+ Provide safe, efficient, patient-centered, evidence-based medication services.
+ Verify medication orders and/or dispenses medications accurately according to state and federal regulations.
+ Document services in a complete, accurate and timely manner.
+ Actively contribute to medication safety initiatives.
+ Ensure all pharmacy operations are in compliance with national accreditation standards.
+ Directs staff appropriately to provide safe and efficient care and maintain professional working relationships with all caregivers.
+ Educate and train students, residents, other caregivers and/or patients.
Minimum qualifications for the ideal future caregiver include:
+ PharmD or Bachelor's of Science in Pharmacy from an accredited college of Pharmacy
+ Current State licensure as a Registered Pharmacist (RPh) or eligible for reciprocation in current state within 90 days of hire
+ For individuals required to administer immunizations,Basic Life Support (BLS) through American Heart Association (AHA) or American Red Cross and completion of a Board of Pharmacy approved course in the administration of immunizations or certification obtained within 90 days of hire are required. For individuals required to respond to adult or pediatric cardiopulmonary arrest events, current Advanced Cardiovascular Life Support (ACLS) or Pediatric Advanced Life Support (PALS) Certification through American Heart Association (AHA), or certification obtained within one (1) year of hire, is required.
Preferred qualifications for the ideal future caregiver include:
+ Board Certification
+ Prior hospital-based pharmacy experience
+ Completion of a Post-Graduate Year 1 Residency (PGY1), fellowship or relevant post-graduate pharmacy experience
+ Post-graduate year 2 residency (PGY2) or relevant experience preferred for Clinical Pharmacists hired in a Specialized Pharmacy practice area (i.e. Critical Care, Oncology)
**Physical Requirements** :
+ Typical physical demands include ability to perform the job while standing and walking for prolonged times, approximately 75% of shift and continuously for extended periods of time.
+ Manual dexterity sufficient to perform computer and dispensing functions of the job; ability to lift up to 30 lbs.
+ Position often includes reaching, stretching, stooping, climbing steps and sitting.
+ Must have normal or corrected vision.
+ Must have ability to clearly communicate verbally by phone or in person
**Personal Protective Equipment** :
+ Follows standard precautions using personal protective equipment as required.
**Pay Range**
Minimum Annual Salary: $101,900.00
Maximum Annual Salary: $155,390.00

A successful candidate's actual compensation will be determined after taking factors into consideration such as the candidate's work history, experience, skill set and education.
