Clinical Pharmacist - Medication Safety

The Florida Research & Innovation Center (FRIC) focuses on basic and translational research related to infectious diseases, inflammatory diseases and cancer. The Clinical Pharmacist in this role will be a key player in maintaining medication safety efforts for the Florida market. You will work to identify and facilitate medication safety improvement projects as well as serve as an internal consultant on medication safety, safety improvement methodologies and medication accreditation compliance strategies. As co-chair of the CC P&T Medication Systems Subcommittee and chair of the CC Patient Safety Committee Medication Safety Subcommittee, you will provide formal and informal education to leadership and staff as well as attend medication-related sentinel events and root cause analysis meetings. Overall, you will be contributing to a culture of safety while working for a world-class organization. **A caregiver in this position works days from 8:00AM - 4:30PM.** A caregiver who excels in this role will: + Identify, facilitate and define priorities for medication safety improvement projects + Assist process or quality improvement teams as needed + Participate in executive walk-arounds to patient care areas to identify latent conditions leading to medication safety problems. + Lead process improvement efforts designed to reduce medication errors or improve patient safety. + Receive, collate and analyze medication safety data from internal/external sources. + Develop and maintain CCHS' medication error reporting screens in SERS and CCHS' electronic interface with MEDMARX. + Ensure appropriate use of medication safety reporting tools and methodologies. + Share and disseminate education, information and best practices regarding medication safety. + Assist with the development of policies and procedures related to managing medication safety and accreditation compliance issues. + Assist with implementing a culture of shared individual and system accountability (e.g., Just Culture). + Provide safe, efficient, patient-centered, evidence-based medication services. + Verify medication orders and/or dispenses medications accurately according to state and federal regulations. + Document services in a complete, accurate and timely manner. + Ensure all pharmacy operations are in compliance with national accreditation standards. Minimum qualifications for the ideal future caregiver include: + PharmD or Bachelor's of Science in Pharmacy from an accredited college of Pharmacy + Current State licensure as a Registered Pharmacist (RPh) or eligible for reciprocation in current state within 90 days of hire + One year of inpatient pharmacy experience Preferred qualifications for the ideal future caregiver include: + Board Certification + Completion of a post-graduate year 1 residency (PGY1), fellowship or relevant post-graduate pharmacy experience Our caregivers continue to create the best outcomes for our patients across each of our facilities.
