Job Summary Clinical Pharmacist Internal Medicine Forsyth Medical Center, Winston-Salem *20K SIGN ON BONUS ELIGIBLE!* Job Summary: An internal medicine clinical pharmacist provides expert care for patients with diverse medical and surgical conditions in the acute care setting, collaborating with healthcare professionals on rounds, managing medication therapy, providing patient education, and optimizing treatment plans for both acute and chronic conditions. Schedule – rotating 1st and 2nd shift – 2 weeks of 1st shift and 1 week of 2nd shift, every 3rd weekend (1st shift – 0700-1530, 2nd shift – 1230-2100) Responsibilities A Pharmacist is responsible for performance and/or supervision of various aspects of the medication use system including but not limited to therapeutic recommendations, advocate for patient safety, preparation, distribution, monitoring, oversight of administration, and billing. This pharmacist will be primarily based on the Internal Medicine Team and may cross-cover other areas as needed. The pharmacist’s focus is on the quality of care through comprehensive medication management. Medication Management: Internal medicine pharmacists are responsible for managing drug therapy for individual patients, identifying and mitigating drug-related problems to ensure safe and effective medication use. Patient Care: Delivers patient-centered care for adults with a wide range of diseases by collaborating with members of the patient’s healthcare team. Patient Education/Transitions of Care: Assists with medication reconciliation, patient counseling, reviewing, and providing education to patients and their families about new medications, potential side effects, and proper medication use prior to discharge. Consultation: Provide expert advice on medication selection, dosage, and administration routes to other healthcare providers. Quality Improvement: Participate in medication safety initiatives, develop clinical guidelines, and conduct research related to medication use in hospital. Qualifications Residency training PGY1 or comparable acute care experience required. Experience: Postgraduate year 1 residency or fellowship OR two years of experience as a registered pharmacist in healthcare system pharmacy required. Bachelor's Degree in Pharmacy, Foreign Pharmacy Graduate Equivalency Committee (FPGEC) Certification, required. Doctorate of Pharmacy, preferred. Experience: Postgraduate year 1 residency or fellowship OR two years of experience as a registered pharmacist in health system pharmacy. Oncology experience preferred for oncology roles, required. ASHP accredited PGY2 Infectious disease residency OR 2 years of equivalent clinical pharmacy experience in infectious diseases/antimicrobial stewardship, required. Licensure/Certification: Current Pharmacy licensure in appropriate state(s) or must be completed within 30 days of start date by either examination or reciprocity, required. Board of Pharmacy Specialties Certification, required. Additional Skills (required): Other duties as outlined in Novant Health and facility specific policies and procedures.
