The Clinical Pharmacist – Inpatient PRN supports the inpatient pharmacy team at Medina Hospital, providing comprehensive clinical and operational pharmacy services across a community hospital with a strong focus on orthopedic care. Working collaboratively with pharmacists, clinical specialists, physicians, nurses, and other members of the interdisciplinary care team, this role helps ensure safe, effective, and evidence-based medication therapy. Responsibilities include verifying and processing medication orders, performing clinical chart reviews and medication reconciliation, entering and clarifying medication orders within the electronic health record, and responding to medication-related questions from caregivers throughout the hospital. The pharmacist also evaluates medication appropriateness, resolves formulary and therapeutic issues, and partners with the care team to optimize patient outcomes while supporting efficient inpatient pharmacy operations.
This is a PRN role. A caregiver in this position will work a variety of shifts between 6:30am and 9:30pm based on departmental needs. Standard shift times include 6:45am–3:15pm, 8:00am–4:30pm, and 1:00pm–9:30pm. Caregivers are required to work at least every fourth weekend, with additional shifts available to provide coverage for PTO and other staffing needs. There are no on-call or holiday requirements for this position.
A caregiver who excels in this role will:
Provide safe, efficient, patient-centered and evidence-based medication services. Verify medication orders and/or dispense medications accurately according to state and federal regulations. Document services in a complete, accurate and timely manner. Ensure all pharmacy operations comply with national accreditation standards. Maintain professional interactions with patients to facilitate successful patient experiences. Educate and train students, residents, other caregivers and/or patients.Minimum qualifications for the ideal future caregiver include:
PharmD or Bachelor’s of Science in Pharmacy from an accredited college of Pharmacy Current State licensure as a Registered Pharmacist (RPh) or eligible for reciprocation in current state within 90 days of hire Current Basic Life Support (BLS) Certification through the American Heart Association (AHA) or American Red Cross and completion of a Board of Pharmacy approved course in the administration of immunizations or certification within 90 days of hire for individuals administering immunizationsPreferred qualifications for the ideal future caregiver include:
Board certification Completion of a post-graduate year 1 residency (PGY1), fellowship or relevant post-graduate pharmacy experience preferred.Post-graduate year 2 residency (PGY2) or relevant experience preferred for Clinical Pharmacists hired in a Specialized Pharmacy practice areaPhysical Requirements:
Typical physical demands include ability to perform the job while standing and walking for prolonged times, approximately 75% of shift and continuously for extended periods of time.
Manual dexterity sufficient to perform computer and dispensing functions of the job; ability to lift up to 30 lbs.
Position often includes reaching, stretching, stooping, climbing steps and sitting.
Must have normal or corrected vision.
Must have ability to clearly communicate verbally by phone or in person
