Clinical Pharmacist II - Pharmacy - DLK Cancer and Infusion Center - Full Time - Days
Located in Fremont, Ohio, ProMedica Memorial Hospital has served the citizens of Sandusky County and the adjacent communities since 1918. POSITION SUMMARY The Clinical Pharmacist II provides advanced pharmacy services, including drug information, pharmacokinetics, drug therapy monitoring, patient education, medication reconciliation, transition of care evaluations, and ensures compliance with controlled substance regulations. Responsible for directly supervising pharmacy support personnel and actively engaging and collaborating with all hospital personnel. ACCOUNTABILITIES *All duties listed below are essential unless noted otherwise* Clinical Pharmacy Practice Performs all duties of Clinical Pharmacist I, including medication order verification, preparation, and dispensing. Reviews targeted patient profiles and recommends evidence-based medication therapy regimens and monitoring plans, ensuring cost-effective therapeutic alternatives are considered. Monitors medication therapy regimens for contraindications, interactions, adverse reactions, appropriate dosing, and therapeutic duplications. Conducts patient education and counseling for inpatients and outpatients as requested. Collects medication history for inpatients and provides recommendations for revisions to discharge medication lists as directed. Contributes to antimicrobial stewardship activities, ensuring appropriate use of antibiotics. Participates in anticoagulant stewardship to enhance safety and efficacy of therapy. Verifies chemotherapy and parenteral nutrition orders, including validation of compounded items. Verifies pediatric orders and compounded items with precision and clinical appropriateness. Drug Information and Escalation Assists in addressing drug information requests escalated from Clinical Pharmacist I and appropriately escalates more complex issues as necessary. Provides guidance in evaluating evidence-based literature to support drug therapy decisions. Supervision, Oversight, and Training Assists in training and mentoring pharmacy staff, fostering skill development and clinical excellence. Supervises and instructs pharmacy students during experiential rotations, ensuring high-quality educational experiences. Maintains effective communication mechanisms and contributes to constructive problem-solving within the team. Utilizes a team-based approach to decision-making and problem resolution, fostering trust and respect among colleagues. Operational Support and Additional Responsibilities Supports daily pharmacy operations to ensure quality patient care and operational efficiency. Meets standards of behavior and builds successful collaborative relationships with team members. Performs other duties as assigned to meet departmental needs. Other duties as assigned REQUIRED QUALIFICATIONS Education: PharmD degree or BSPharm with relevant experience post- 2006. Skills: Experience in hospital pharmacy services, including unit dose, IV admixtures, medication reconciliation, and addressing provider questions. Years of Experience: Minimum 1 year (full-time equivalent of 2,040 hours) in inpatient pharmacy practice. License: License: Must be a registered pharmacist, licensed in good standing in Michigan and Ohio. Single facility based pharmacists must retain state licensure per site location. Certification: BLS (Basic Life Support) PREFERRED QUALIFICATIONS Education: Completion of a PGY1 Pharmacy Residency Training Program. Skills: Expertise in patient counseling, pharmacokinetic evaluations, preceptorship, antimicrobial stewardship basics, evidence-based literature review, and time management. Experience in leadership roles or multidisciplinary team collaboration is highly desirable. Years of Experience: Minimum 3 years (full-time equivalent) in inpatient pharmacy practice. License: no additional preferred Certification: ACLS (Advanced Cardiac Life Support), Board Certification, or two professional training certificate programs for acute care. WORKING CONDITIONS Personal Protective Equipment: Required when working with patients or hazardous medications. Physical Demands: Must be able to lift light materials. Non-material handling of continuous sitting and reaching; frequent standing; occasional squatting, kneeling, walking, and stairs. Must be able to move about and between workstations.
