Clinical Pharmacist - Chronic Pain Management
LOOKING FOR A CLINICAL PHARMACIST TO JOIN OUR INTERDISCIPLINARY CHRONIC PAIN TEAM Job Summary: In collaboration with physicians and other members of the care team, our clinical pharmacists provide a wide array of clinical pharmacy services, ranging from primary care to targeted high-risk, high-cost specialty care. They care for diverse patient populations across all lines of business. The majority of patient care is provided virtually via telephone, however, decentralized clinical pharmacists can work with patients face-to-face in the clinic. Sample patient care activities include: Collaborative Drug Therapy Agreements (CDTA) in various therapeutic areasPrimary Care Chronic Disease Management (CDM): asthma, diabetes, ASCVD, and hypertensionSpecialty CDM opportunities: Hepatitis C, HIV/PrEP, MS Medicare Medication Therapy Management (MTM) Program Medication consults to primary care and specialty care teams Hospital discharge medication reconciliationMedication Use Management (MUM) - initiatives to improve affordability and/or safety Quality - initiatives to improve patient care and organizational performance on HEDIS and CMS 5-Star measures Essential Responsibilities: + Support statewide clinical pharmacy services in partnership with key stakeholders to provide medication management of targeted patient populations, disease states, and high cost medications. Provide clinical consults and drug educations for complex patients. Accountable for affordability, quality and productivity targets specified by Pharmacy department + Work with clinical teams to identify and develop quality/safety initiatives. Provide drug education for local clinical staff and ensure integrated communication. + Support development of clinical education for assigned areas and support training to ensure staff achieve clinical/standard work competencies (peer review) and professional development targets. Provide support as clinical content expert/leader and/or operational program performance leader as specified. + Lead or support regional leadership and/or external stakeholders to innovate and identify new opportunities and ways to improve performance and growth. Basic Qualifications: Experience + Minimum three (3) years of clinical pharmacy experience in multidisciplinary team environment or completion of a one year residency. Education + Bachelors degree in Pharmacy or PharmD. License, Certification, Registration + Pharmacist License (Washington) required at hire + Board of Pharmacy Specialties Board Certification within 24 months of hire Additional Requirements: + Excellent verbal and written communication skills (including presentations) + Experience in educating physicians and other members of the care team + Strong medication evaluation skills and ability to apply knowledge to patient care + Strong time-management skills, ability to prioritize workload, and optimize efficiency + Ability to problem solve and innovate + Work effectively in team-based environment + Critical analysis skills - able to evaluate data, interpret findings, and clearly communicate observations/recommendations to target audience (e.g., physicians, nurses, patients, leadership) + Excellent customer service skills and patient-centered orientation + Strong leadership skills and ability to lead/influence teams + Up-to-date knowledge of pharmacotherapy + Fluent with Microsoft Office Suite (Word, Excel, PowerPoint)Able to seek out additional tools and resources as needed. + Must have excellent customer service skills. + Experienced clinical leader in working with interdisciplinary teams. + Must have strong medication evaluation skills and ability to apply to patient care. Preferred Qualifications: + Clinical pharmacy experience in the ambulatory care / clinic setting. + Doctorate degree in pharmacy. + Completion of PGY1 and/or PGY2 residencies OR equivalent professional experience.
