Clinical Pharmacist - Cancer Hematology Clinic
What We Offer HYBRID POSITION The position resides within a federally designated Hemophilia Treatment Center; it is a life span center (adult & pediatric population) The Pharmacy Vision is to deliver optimal medication management across all dimensions of care, every time, and that each Pharmacy employee understands how he or she contributes to the health system’s vision of achieving that commitment to patients and families. Each Pharmacy employee will be essential in ensuring that Novant Health Pharmacy exists to ensure safe, effective, and affordable medication use in our communities, one person at a time. There will be clinical and operational responsibilities: Collaborate with adult and pediatric hematologist to monitor and to interpret blood levels of factor and making dosing recommendations/adjustments. Calculation of individual patient pharmacokinetics for factor products Provide dosing recommendations for treatment of spontaneous bleeds or surgical prophylaxis Management/monitoring of factor refills and identification/management of barriers to medication adherence Collaborate with providers to evaluate inpatient factor use/provide continuity of care upon discharge for hospitalized HTC patients. Operational Audit patient eligibility for dispensation via contracted HM 340 B specialty pharmacies Assist with internal process monitoring of factor products dispensed through contracted HTC specialties pharmacies Assist patients/collaborate with nurse navigators related to insurance issues (prior auth and copay assistance) Coordinate ordering/shipping between specialty pharmacy and patient Participate in research and quality improvement Provide ongoing education of advanced practice providers and nursing team related to bleeding disorders and medication therapies Collaborate with HTC Program Director to develop/maintain HTC program policies and procedures What We're Looking For Education: 4 Year/Bachelor's Degree in Pharmacy, required. Doctorate of Pharmacy, preferred. Experience: Postgraduate year 1 residency or fellowship OR two years of experience as a registered pharmacist in health system pharmacy, required. Licensure/Certification/Registration: Current Pharmacy licensure in appropriate state(s) or must be completed within 30 days of start date by either examination or reciprocity, required. Board of Pharmacy Specialties Certification or other approved certification must be obtained and continuously maintained within 2 years of start date as applicable to the role; excludes PRN team members who may not be eligible to take certification exam, required. Additional Skills Required: Other duties as outlined in Novant Health and facility specific policies and procedures. Ability to drive/travel to multiple facilities/locations as needed.
