DESCRIPTION
As a member of the multidisciplinary health care team, the Transplant Clinical Pharmacy Specialist will provide the full range of pharmaceutical services to support the needs of the Newark Beth Israel Medical Center Heart Transplant and Lung Transplant Programs. Under the direction of the Assistant Director of Clinical Pharmacy Services, the Transplant Clinical Pharmacy Specialist will be responsible for reviewing and optimizing medication therapy for transplant patients in collaboration with other health care practitioners. Pharmacist will actively promote medication safety through the identification and analysis of adverse drug events and medication incidents, developing and implementing evidence-based and or cost-savings initiatives, and providing educational expertise and support within the hospital when required to meet the goals of the organization.
QUALIFICATIONS

Qualified candidates must have a Doctor of Pharmacy degree from an ACPE-accredited school of pharmacyNJ license as a Registered PharmacistCompletion of PGY1 Pharmacy Residency trainingCandidates should be self-motivated, have good communication, organizational, and interpersonal skillsPreferred
Board Certification in Solid Organ Transplantation PharmacotherapyCompletion of PGY2 Pharmacy Residency training in Solid Organ Transplant Pharmacy or a minimum of 3years experience in transplant pharmacy practice.RESPONSIBILITIES
The major job responsibilities of this position are to:
Provide pharmaceutical care to patients evaluated by the Heart Transplant or Lung Transplant service in the inpatient and or ambulatory care settingServe as a pharmacotherapy resource for licensed independent practitioners, nurses, and other healthcare disciplines Educate patients and various healthcare disciplines on appropriate medication use in the transplant patient populationSupport the PGY1 Pharmacy Residency Program and other department and hospital initiatives as needed Ensure compliance with all hospital and regulatory requirements for Heart and or Lung Organ Transplant ProgramsThe following activities are components of the position to assist in meeting the major job responsibilities:
Participate in daily patient care activities and optimize pharmacotherapy management in collaboration with other members of the multidisciplinary Heart and or Lung Transplant team.Provide education and counseling to transplant patients regarding their medication useMonitor drug therapy to evaluate appropriateness of use, dosing regimen, and potential for medication safety concerns. ex: therapeutic duplication, drug interactions. Provide pharmacotherapy consultations and other medication-related support as requested.Prevent, detect, monitor, document, and report adverse drug reactions and medications errors.Serve as a primary preceptor for the PGY1 Pharmacy Practice Residency Program and participate in the overall teaching and mentorship of pharmacy residents.Benefits and Perks:
