Clinical Pharmacist – Ambulatory, Specialty Pharmacy

Work Schedule:

100% FTE, Monday – Friday, 7:00 am – 6:00 pm (8 hour shifts), but hours may vary based on the operational needs of the department. Includes weekends/on-call. This is a hybrid position.

We are seeking a Pharmacist – Ambulatory Specialty Pharmacy to:

Provide medication therapy management services to assure that drug therapy is safe, effective, efficient, and cost-effective.Provide chronic disease state management and educations as part of interdisciplinary team.Assist with medication access and affordability services.Act independently within the scope of their license and areas of expertise with the help of general policies, procedures and practice guidelines.Collaborate with leadership to ensure strategic goals for the department are achieved.

UW Health Pharmacy Services Building – a non-clinical location in Middleton that houses nearly 100 Pharmacy professionals. These employees perform various services and duties, including pharmacy administration, Pharmacy Technician Training Program, pharmacy primary care services, medication prior authorization, sterile and non-sterile compounding, specialty/mail service program and call center.
