Schedule: Full-Time, Day Shift Position OverviewThe Clinical Nurse - Navigator assesses patient needs and develops personalized care plans, partnering with patients, primary physicians, and multidisciplinary teams. This role advocates for the patient, facilitates communication, and ensures that appropriate tests and treatments are delivered efficiently and effectively. The Navigator balances clinical needs with resource management, promoting high-quality, patient-focused care.This position is dedicated to supporting our Theranostic Program, providing specialized navigation and coordination for patients undergoing theranostic treatments.Key Responsibilities:Evaluate the care needs of assigned patients and develop comprehensive, individualized care plans.Collaborate with physicians, nurses, social workers, and other healthcare team members to coordinate timely and appropriate care.Serve as an advocate for patients, ensuring their needs and preferences are prioritized.Monitor patient progress and modify care plans as needed.Facilitate effective communication among care team members and with patients and families.Support resource utilization to ensure efficient, high-quality care delivery.Educate patients and families about their care plans, treatments, and available resources.Qualifications & Requirements:Education:Associates of Nursing (ASN) required.Bachelors of Nursing (BSN) preferred.Licensure:Active RN license in Indiana or Nurse Licensure Compact (NLC).Graduate of a nationally accredited nursing program.Certifications:Basic Life Support (BLS) certification through the American Heart Association (AHA).Additional advanced life support certifications may be required per department policies.Experience:3-5 years of clinical nursing experience preferred.Experience in care coordination, case management, or patient navigation is a plus.Skills & Knowledge:Familiarity with medical terminology, medical record standards, and healthcare regulations.Strong computer skills, including word processing and database management.Excellent communication, advocacy, and problem-solving skills.Why IU Health University Hospital?As part of Indiana’s largest and most comprehensive healthcare system, we offer: Competitive salary and comprehensive benefitsOpportunities for professional development and growthA collaborative, innovative environment dedicated to excellenceSupport for delivering coordinated, patient-centered careIf you'd like to learn more about our benefits, please visit: IU Health BenefitsMake a meaningful difference in patient care—apply today to join IU Health University Hospital as a Clinical Nurse - Navigator!
