intake department including performance management and process improvementEDUCATIONCERTIFICATIONGraduate of an NLN accredited school of nursing BSN preferredPossession of a current license to practice as a registered nurse in the State of ConnecticutEXPERIENCETwo yearsrsquo full time clinical experience at least one of which was in a home care agencyCOMPETENCIESKnowledge of Agency policies and principlesCapable of implementing Agencyrsquos goals and directionsKnowledge of communities servedCompetent in clinical health principlesKnowledge of state and federal regulations and accreditation standardsAbility to work positively with staff peers subordinates and superiorsAbility to function independently within scope of responsibilityESSENTIAL DUTIES and RESPONSIBILITIESDisclaimer Job and NTUs on shared boardAssures that the highest quality and professionalism is provided to all customersOversees day to day function of Intake nursesResponsible for identifying and obtaining all necessary information for referralsAssess W and all documentation from referral sources for appropriateness for home care servicesObtains verbal order for SOC that includes a description of the patientrsquos condition and the services to be provided by the AgencyAssigns accurate ICD codes to referral diagnosisObtains history and physical and other documentation required for codingCoordinate all disciplinesservices for successful initial assessmentObtains all necessary documentation for face to face documentResponsible for establishing and maintaining positive relationships with referral sources within the hospital physician offices or healthcare facilitiesInformation resource for home health servicesAssist complex patient conferencing as neededResource for early intervention need for cooperative relationshipsServes as a liaison between referral sourcesAssist with payor information and verification processPerforms other duties as assignedResponsible for overseeing and managing the
