Clinical Educator

The Clinical Educator (CE) provides education and training to assigned clinicians at new hire and as needed for current staff. The Clinical Educator will follow up with newly trained staff to ensure and validate training and education is implemented accordingly. In this role, you will report to the Director of Nursing/Director of Patient Care Services.


Duties/Responsibilities:

Responsible for mentoring, educating and evaluating a group of qualified clinicians and caregivers

Conducts training classes on a regular bases for the following but not limited: new hire orientation, skill advancement, aide training, newly implemented clinical process and re-education as needed

Responsible for initial education and on-going mentorship of new clinical staff

Assists clinical team in achieving goals as assigned regarding outcomes, productivity, re-hospitalization, and QAPI and care coordination

Participates in in-service programs, supervising and teaching other nursing personnel

Participates in activities relative to the provision of professional services to an assigned group of patients as applicable/assigned

Acts as back-up and additional support for all Clinical Leadership functions as needed

Provides direct patient care and all case management functions per policy when necessary

Provides all care in accordance with the Plan of Care

Assists to ensure that all staff providing care maintain professional standards of community nursing practice

Maintains and adheres to agency procedure and patient care policy standards

Assists in overall coordination of patient care services for an assigned group of patients in association with clinical team

Educates field staff to ensure all case management is provided as described in state regulations

Assists with education and implementation of processes for case assignments and case management

Assists to educate appropriate staff about home health statutes and administrative rules

Assists with installation of improved work methods and procedures to ensure achievement of objectives of the Agency

Performs competencies and core competencies. Educates as applicable to ensure optimal success by field staff

Viewed as an expert clinical resource to a group of clinicians

Assists in oversight clinically of: making patient and personnel assignments, coordinating patient care, coordinating referrals, assuring that patient needs are continually assessed, and assuring the development, implementation, and update of the individualized care plan

Informs clinical staff as assigned regarding policies/processes of the Agency

Reports/assists the regarding changes that might affect the clinical operations of the location

Performs coordination as assigned during emergency situations and understands the Emergency Plan

Maintains confidentiality of agency, employee, and patient matters

Responsible for clinical orientation of new staff prior to patient contact

Assists in development and administration of on-going continued education efforts

Participates in Performance Improvement activities as requested

Collaborates with the QA staff on review of clinical documentation and educates staff to proper documentation to meet regulatory and billing requirements

Assists with staff performance appraisals

Educates field staff to billing processes as applicable from clinical perspective

Attains annual goals as established by Clinical Leadership in the annual performance evaluation

Maintains balance between field time and in-office time as directed by the Clinical Leadership to ensure all responsibilities are met

Submits documentation within required timeframes

Performs other duties as assigned by Clinical Leadership


Required Skills/Abilities/Knowledge:

Excellent computer skills including current devices and programs being utilized in the field

Known as a clinical field resource for clinical tasks and critical thinking projects

Possesses a commitment to be available during scheduled orientation dates

Demonstrates knowledge, comprehension, and compliance with all state and federal laws, rules, regulations and agency policies and procedures. If agency is accredited, understands the accreditation standards

Able to work and communicate with a diverse group of clinicians and communicate to all levels of local authority

Maintains all HR requirements, competencies, professional continuing education, and any other required items

Desire to coach, guide and empower orientee to achieve optimum success

Ability to work in a fast-paced challenging environment with time demands and shifting priorities

Excellent interpersonal communication skills

Excellent time management skills to plan, organize, implement and evaluate the orientee(s) successfully

Self-directed and shows interest in new experiences

Able to take on-call duty nights, weekends and holidays as assigned, if applicable


Education/Experience/Licenses/Certifications:

Minimum of three to five years of clinical experience of which at least two years are home health experience with an excellent understanding of clinical processes and procedures

Solid understanding of all OASIS questions and interpretations as stated in Chapter 3 of the OASIS Implementation Manual

Successful experience in a mentoring/coaching role(s) (preferred). Possesses desire to work with and teach new employees

Possesses clear professional license, background screen and fingerprint card, if applicable

Cannot have any verbal/written discipline actions within the last six months - one year if hired internally

Pay Range: $70,000 - $80,000 / salary
