Description:This position's primary role is to provide Medical Nutrition Therapy (MNT)and Diabetes Self-Management Training (DSMT) to adults in the outpatient setting. Provides 1:1 and class education. Assesses nutrition status of assigned patients and implement all components of the nutrition care process, according to food and nutrition department policies and local regulatory standards (IDPH, TJC, CMS, etc.). Monitors and evaluates nutrition interventions, makes appropriate adjustments to plans, as needed, and reports results of quality outcomes in the inpatient and outpatient setting. Has specialty practice experience and working knowledge in diabetes management. This position is a part of the Clinical Career Ladder Program.Assess, interpret, plan, monitor and document the nutritional status and nutritional needs of patients using evidence-based standards of care, policies and practice guidelines. Perform a nutrition-focused physical exam on patients. Identify and provide age-specific nutrition care to meet the cultural needs of the patients and their families. Work within an inter-disciplinary team to provide a plan of care to achieve the patient's medical goals. This position also manages and coordinates medical nutrition therapy of complex, high acuity patients within the dietitian's area of specialty practice. Educate patients, care givers, students and other health care colleagues on the effect of therapeutic diets and nutrition support to maintain/improve nutritional status.Demonstrate leadership skills by presenting on nutrition topic related to their expertise. Example of settings include journal club, in-service, Grand Rounds, THS-FANS program. This may also include participating on a hospital committee or THS -FANS sub-committee.Participate in department activities to enhance patient satisfaction. Keep current on nutrition research and incorporate into daily practice. Other clinical duties as assigned.

Compensation:
Pay Range: $53,144.00 - $82,368 Annually
Actual compensation will fall within the range but may vary based on factors such as experience, qualifications, education, location, licensure, certification requirements, and comparisons to colleagues in similar roles.
