The position will coordinate all anticoagulation pharmacy services across the enterprise. The organization provides continuous education, leadership development, and mentorship programs to advance pharmacists in their careers. With a robust clinical research infrastructure, pharmacists have opportunities to engage in clinical trials, quality improvement projects, and evidence-based initiatives.
**Caregivers in this role will work Monday through Friday from 7:00am to 4:00pm. Weekend rotations will be every eighth weekend. No holiday or formal on call expectations.**
A caregiver who excels in this role will:
+ Identify needs and implement standardized clinical pharmacy services across pharmacy service lines (e.g. Anticoagulation).
+ Provide leadership to service line by proactively assisting in high level clinical services, training, and education.
+ Provide mentoring to pharmacists, residents and students.
+ Provide ideas regarding new clinical services, cost savings initiatives, and practice innovation through active participation in strategic planning efforts.
+ Establish processes to evaluate quality assurance within service line.
+ Identifies pharmacy representation for academic responsibilities and interdisciplinary committees.
+ Participate in interviewing and recommends selection of candidates.
Minimum qualifications for the ideal future caregiver include:
+ Doctor of Pharmacy degree from an American Association of Colleges of Pharmacy (AACP) accredited university
+ Current state licensure as a Registered Pharmacist (RPh) or eligible for reciprocation in current state within 90 days of hire
+ For individuals required to administer immunizations, current Basic Life Support (BLS) certification through American Heart Association (AHA) and completion of a Board of Pharmacy approved course in the administration of immunizations or certification obtained within 90 days of hire
+ For individuals required to respond to adult or pediatric cardiopulmonary arrest events, current Advanced Cardiovascular Life Support (ACLS) or Pediatric Advanced Life Support (PALS) Certification through American Heart Association (AHA), or certification obtained within one (1) year of hire
+ Completion of a post-graduate year 2 residency (PGY2) plus 1 year of experience providing pharmaceutical care in the relevant specialty area or
+ Completion of a post-graduate year 1 (PGY-1) residency plus 3 years of experience providing pharmaceutical care in the relevant specialty area or
+ 5 years of experience providing pharmaceutical care in the relevant specialty area
Preferred qualifications for the ideal future caregiver include:
+ Board certification through the Board of Pharmacy Specialties that aligns to practice area strongly preferred required within one year
+ Previous leadership experience
