Position Summary / Career Interest:The Clinical Case Manager Lead, under the direction of the Clinical Case Management Manager, is a department leader who collaborates with the healthcare team to ensure safe, efficient, evidence based care/service to patients of all ages and their families. Primary role is to lead, motivate and train the case management team. Responsible for developing, implementing and updating department training resources to reflect changes within case management role. Organizes new employee orientation by scheduling preceptors, intermittent follow-up and providing oversight of orientation and competency of staff. Educates all staff (including weekend and PRN team) as it relates to department functions, processes, regulatory requirements and systems appropriate to the position. Trains and mentors new case management staff. Creates and maintains a supportive work environment for the entire team.

Responsibilities and Essential Job FunctionsAccepts responsibility and accountability for achievement of optimal outcomes within their scope of practice. Follows policies, procedures and standards; complies with Corporate Compliance program. Assumes responsibility for risk and safety issues associated with the position. Takes call as required by the department expectations. Performs specific job responsibilities and demonstrates accountability for own actions and decisions. Collaborates with leadership and departmental staff regarding changes that impact case management’s role. Acquires and maintains knowledge and competence related to the expectations of their position and practices within their scope. Brings ideas and concerns to supervisor, participates in department decision making. Demonstrates flexibility and teamwork among case management staff members. Assists peers in the event of fluxuating census. Provides coverage to other services as needed or as requested by Manager Assists with hiring, education and performance evaluation of the staff Identifies, monitors and reports opportunities for quality improvement to the appropriate department Ad-hoc member of department practice council Facilitates ongoing unit education and professionalism through the provision of in-services, CEU opportunities, promoting national certification, models evidence-based practice Participates in the development and review of policies and procedures The CLINICAL CASE MANAGER LEAD takes an active role in performance improvement activities as it relates to their area of assignment. Evaluates training effectiveness and responds as appropriate Participates in department compliance monitoring and auditing Works with the manager and human resources in performance management counseling and discipline of staff Must be able to perform the professional, clinical and or technical competencies of the assigned unit or department. Required Education and ExperienceBachelors Degree in Nursing OR Masters Social Work OR 2 or more years case management/care coordination experience
Preferred Education and ExperienceMaster Degree Nursing or related field health care experience
Required Licensure and CertificationLicensed Registered Nurse (LRN) - Single State - State Board of Nursing Registered Nurse in State of Kansas OR Licensed Master Social Worker (LMSW) - State Division of Professional Registration in State of Kansas OR Basic Cardiac Life Support (BLS or BCLS) - American Heart Association (AHA)
Preferred Licensure and Certification Case Management certification
