Responsibilities
Collaborate with providers to develop and implement standards of care for select patient populations.Functions as a liaison between staff, physicians, social workers, mental health staff, dieticians, and patients/families for patients by assessing, securing, and coordinating appropriate services regarding weight loss care. Evaluates patient care according to pre-established patient care standards.Establishes nutrition protocols for post-operative patients.Coordinates post-operative follow up care in the outpatient clinic. Collects and organizes data from patient care paths, clinical documentation, contactswith patients.Other duties as assigned by management.
Qualifications
Required Education and Experience
Graduate of an accredited school of nursing requiredRequired Licensure and Certifications
Registered Nurse: North Dakota or Compact License required.American Heart Association Basic Life Support Card - CPR required.Required Minimum Knowledge, Skills, Abilities and Training
Language: Ability to read and comprehend written and oral communication.Math: Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions and decimals.Reasoning: Ability to anticipate, recognize and resolve problems.Computer skills: Ability to operate and navigate applicable computer programs.Clinical ability: Ability to demonstrate competency in job related functions according to organizational standards.Preferred Education and Experience
Bachelor of Science in Nursing preferred1 year related experience in a clinic, specialty clinic or acute hospital setting preferred2 years experience in obesity disease management and/or weight loss surgery preferredOverviewAs out Clinic Bariatric Coordinator RN, you will provide comprehensive care coordination for patients, families, and caregivers by serving as the designated contact, facilitating interactions with the clinical care team, and providing seamless navigation through the health system.
