Position Location: Colorado Mental Health Hospital in Pueblo

About the Position:
This position works under the supervision and direction of a registered nurse and assists with psychiatric and medical nursing care of mentally ill patients; observes, reports, and documents patients’ behavior, physical condition (including vital signs, blood pressure, weights, patient reports of pain, etc.), and reactions to medications and treatment; and provides input into planning patient treatment plans; monitors and intervenes to ensure a safe and therapeutic patient milieu; and provides immediate crisis intervention when indicated. Provides basic hands-on care in a safe, clean, and infection-controlled manner, including bathing, dressing, grooming, personal hygiene, skin care, range of motion exercises, positioning, ambulation, transferring, toileting, feeding, transportation, and assistance with patients’ laundry, safety checks, and documentation of all the above. Duties in this position include:
Clinical Skills:
+ Under the supervision of a registered nurse, interact with patients to build and sustain a therapeutic relationship that promotes the patient’s psychiatric recovery and mental health. .
+ Utilizes therapeutic interventions to manage crises and manage assaultive and disruptive behaviors.
+ Promptly responds to the patient’s psychiatric and/or medical crisis.
+ Follows and maintains Infection control standards and regulations.
+ Supports and assists patients in accomplishing activities of daily living (ADLs) through monitoring, teaching, supporting, and direct assistance, when needed. Provides direct patient care, including bathing & showering, personal hygiene, shaving, brushing teeth, care of personal items, laundry, toileting, dressing, undressing, assisting in and out of bed, feeding programs, toileting programs, range of motion, transporting, and interacting socially to meet patients’ basic needs
+ Prepares patients by explaining actions and interventions before providing care, whenever possible
+ Responds to duress alarm.
+ Documents observations, care provided, and patient response on flowsheets or other charting formats as assigned and required by hospital policy
+ Maintain professional boundaries with patients and their families.
+ Assures that the patient is treated with dignity and respect, maintaining the patient’s privacy
+ Communicates to provide or exchange information while keeping registered nurses and other staff informed of patient status and patient issues
+ Escorts patients to appointments on the grounds and transports within the community
+ Learns, completes, and maintains competency in patient care, including taking vital signs, patient hygiene, and personal care.
+ Organizes time daily to ensure timely completion of assigned tasks, documents timely at the end of each shift.
Accountability:
+ Understands and adheres to CMHIP standards and guidelines, including Treatment Plans, the Infection Control Manual, the Rainbow book, and Emergency Guides.
+ Follows and upholds infection control standards and regulations.
+ Complies with the CMHIP Employee Code of Conduct and the organization’s Ethics policies.
+ Follows and adheres to all nursing competencies associated with the CCA II.
+ Understands and utilizes the nursing chain of command.
+ Assists RN and MHC to complete precaution levels and accountability sheets.
+ Provides safe and secure transport for patients to locations such as the yard, gym, visits, administrative conference room, staffing and disposition reviews, and court or community reintegration sites.
+ Documents tasks in accordance with Nursing Department standards.
+ Knows how to access and is expected to utilize the nursing drive on the computer.
+ Responsible for ensuring that the educational profile is current and up to date.
+ Participates in the Quality Assurance/Performance Improvement (QA/PI) process at the unit level, as directed by the Lead Nurse.
+ Attends weekly nursing rounds or reviews the information in the weekly Nursing Executive Council (NEC) minutes to stay informed about nursing directions, new information, and any changes to policies, procedures, or instructions.
+ Any incidents of harassment or workplace violence should be reported by alerting public safety, the charge RN, the Lead Nurse, and the nursing appointing authority regarding such claims.
+ Arrives to work on time and prepared to begin the shift at the scheduled time. Clocks in and out for scheduled work hours to prevent any unnecessary unauthorized overtime. Follows policies related to use of leave (Addenda C-9 ; Use of Leave), calling off, and scheduling time off
Communication/Interpersonal Skills/Customer Service:
+ Effectively communicates by actively listening and sharing relevant information with co-workers, supervisors, and customers to anticipate problems and ensure departmental efficiency.
+ Adapts communication methods to suit different audiences, promoting a positive environment. Maintains professionalism in all interactions with co-workers and patients.
+ Receives and reviews cross-shift reports from the outgoing RN and stays informed about all 1:1 patients, SP II status, and any medical or psychological issues affecting each patient.
+ Addresses patient complaints at the unit level and reports any allegations of abuse immediately to the charge RN, lead nurse, or nursing authority, while also referring them to public safety.
+ Alerts the RN to any exceptions or changes in patient status, including suicidal or homicidal ideation, and identifies needs for medication or RN intervention.
+ Completes necessary paperwork, routing it to the appropriate areas or individuals, including incident reports and documentation of injuries on the job.
+ Corrects and reports any hazardous environmental conditions to the charge RN.
+ Treats all patients and staff with respect. Interactions with all persons are positive, demonstrate tact and diplomacy, and encourage open communication and mutual trust.

Minimum Qualifications, Substitutions, Conditions of Employment & Appeal Rights
Minimum Qualifications:
A high school diploma or General Educational Diploma (GED)
AND
One year (1) experience in an mental health or inpatient hospital setting
Substitutions:
Current, valid certification as a Nurse Aide (CNA) from the Colorado Department of Regulatory Agencies will substitute for the high school diploma or GED.
Current, valid licensure as a Practical Nurse (LPN) from the Colorado Department of Regulatory Agencies or any state participating in the Nurse Licensure Compact will substitute for the experience requirement
Preferred Qualifications:
+ Experience working in a mental or behavioral health setting
+ Possession of CNA License
+ Experience assisting residents/patients with their Activities of Daily Living (ADL) care with the mentally ill population.
+ 1 year experience in an inpatient setting
+ Previous State experience
Conditions of Employment:
+ Full Background check. Name check, TRAILS and Fingerprinting, CAPS
+ Must be 21 years of age or older
+ A Tuberculosis Test (TB), and other required immunizations will be required upon hire.
+ May be required to work overtime as needed on a shift or weekend not part of scheduled hours (days, evenings, or nights).

+ Use, handling, or exposure to hazardous materials (may be exposed to bodily fluids or caustic chemicals)

+ Successfully pass all modules of the CMHIP New Employee Orientation core curriculum, include English reading comprehension, relevant competency assessments, and written/practical exams in Defensive Tactics;
+ Possess the physical ability to complete CTI and CPR;
+ Lift arms above head and kick as high as one's own waist;
+ Stabilize another person to accomplish a controlled containment to the floor;
+ Withstand impact on own body using an ASP shield;
+ Rotate body 90 degrees with feet planted for using a defensive tactic;
+ Kneel on one or both knees and stand up again with multiple repetitions;
+ Must be able to see and hear to observe, address, and respond to potential breaches in safety;
+ Seize, hold, control, or subdue violent or assaultive persons and defend oneself or others to prevent injury;
+ Demonstrate body agility, strength of all four limbs, and the ability to walk and climb stairs consistently;
+ Work in situations involving assaultive behavior, physical control of another person, and/or restraint situations;
+ Ability to read and comprehend the English language; effectively and fluently communicates in English, both verbally and in writing;
+ Work in an environment with fluctuating noise and temperature (hot and cold), which may include working outdoors in inclement weather and/or concrete floor/wall work settings;
· Willing and able to travel;
