This unique position will have a particular focus on the individualized care of medically and socially complex patients with extra emphasis on care coordination. Internal Medicine and Internal Medicine-Pediatrics Physicians are encouraged to apply!Position HighlightsJoin Delaware’s premier health system and provide care to a diverse, multi-cultural population representing all ages7 sessions per week of clinical care including private panel and the potential for resident precepting3 sessions per week for educational, population health, care coordination, interdisciplinary meetings and administrative workSmaller patient panel size to balance the increased intricacies involved in the care of a more complex patient panelExtended appointment duration times to support multi-disciplinary appointmentsSupport with dedicated Behavioral Health, PharmD, Social Work and Care Management servicesTeach and evaluate Internal Medicine, Med-Peds, EM/IM residents and medical students in the health system’s largest teaching practice Participate in resident didactics as part of the ambulatory academic half dayMentor resident quality improvement and clinical initiativesParticipate in faculty development sessionsHelp improve outcomes of our highest health care utilizer patientsWhy ChristianaCare?
