Various (United States of America)Job Summary:\rReviews and verifies the record completion status of inpatients, observations, procedural areas and hospital outpatient surgery for WellStar Health System providers.\rResponsible to ensure that all medical record documentation is accurate and thorough in accordance with federal/state law, accreditation standards and hospital policy.\rThe chart deficiency analyst is also responsible for deficiency reanalysis that involves review, research, analysis and correction of chart deficiencies as well as the physician decline workqueues to reassign to the correct provider as necessary. In addition, reviews the \"done deficiencies\" to ensure the provider completed the deficiency appropriately.\rCore Responsibilities and Essential Functions:\rProvider Chart Completion\r* Accesses, reviews, researches and completes the assignments in the Epic workqueue lists to include analysis needed, declined deficiencies, missing documentation, done deficiencies, final analysis, etc.\r* The analysis needed workqueue list is the primary function performed by this position. Verify the auto-created deficiencies are assigned to the correct provider and add additional deficiencies for missing elements according to hospital policy.\r* Researches declined deficiencies to determine which provider the deficiency needs to be validated and if necessary, reassigned to a different provider. This requires significant analytical and problem solving skills.\r* Verbal orders are routed to the nursing pool for review. If they are unable to determine the provider, it is routed to the HIM department for further research and provider assignment. If the order was declined because it needs to be revised, it is reported to the Chart Correction team.\r* Cosign orders are declined by nurse practitioners, physician assistants, midwives, etc. They need to be researched to determine the correct provider for reassignment. If the order was declined because it needs to be revised, it is reported to the Chart Correction team.\r* Note-based deficiencies [history and physicals (H&P), discharge summaries (DC summary), operative notes, progress notes, etc.] are reviewed to determine if the deficiency needs to be reassigned to another provider, sent back to original provider or locate the documentation in other visits.\r* Reviews the final analysis workqueue list as the last step in the chart completion process. Reviews chart for accuracy and completeness according to the medical staff medical record policy. If any required elements (i.e. H&P, DC Summary, etc.) are missing, a deficiency is assigned. If the chart is complete, the account status is changed from open to closed and finalized.\r* Reviews done deficiencies to ensure the provider completed the deficiency appropriately. If the deficiency is not done, it will be reassigned to the provider for completion. If the deficiency is done, complete the deficiency.\rQuality and Productivity\r* Correctly identifies missing elements at least of the time.\r* Assigns deficiencies to physician(s) according to procedure with at least accuracy.\r* Link scanned signature deficiencies to the correct provider, if applicable, at least of the time.\r* Correctly identifies when a History and Physical, Discharge Summary or Operative Report has not been done and assigns to the correct provider at least of the time.\r* Analyzes an average 120 encounters per day.\rPerforms other duties as assigned\rComplies with all Wellstar Health System policies, standards of work, and code of conduct.\rRequired Minimum Education:High School Diploma General or GED General\r\rRequired Minimum License(s) and Certification(s):\rAll certifications are required upon hire unless otherwise stated.\rAdditional License(s) and Certification(s):\rRequired Minimum Experience:\rMinimum 3 years in HIM record processing in an acute care setting or department established equivalent Required and\rIn order to meet the department established equivalent, HIM departmental quality and productivity standards ( quality, productivity) must be met for the most recent 6 months consistently. Required and\rEmployees who are in a disciplinary process (first written, final written or performance improvement plan) are not eligible for Chart Deficiency Analyst until 12 months following their last disciplinary action.
