Spiritual Health Practitioner
The Spiritual Health program at the Atlantic Health System Accountable Care Organizations (ACOs), is currently looking for a dedicated, passionate, and organized Per Diem Spiritual Health Practitioner to join our team.
The Per Diem Spiritual Health Practitioner will work together with our interdisciplinary team to initiate spiritual care relationships with patients, their families, as well as staff as needed in an ambulatory context. In addition to clinical care, the Per Diem Spiritual Health Practitioner will assist patients and their families with referrals to supportive resources within Atlantic Health System and the larger community.
The Per Diem Spiritual Health Practitioner’s principal activities will include but not be limited to:
Providing spiritual care to patients navigating a diversity of healthcare journeys, providing care as they experience serious illness, face complex medical decisions, and/or endure existential and spiritual distress, trauma, and loss. Responding to new patient referrals within three days of receiving them (excluding weekends) Charting in outpatient EPIC or other appropriate platforms, including spiritual health assessments and spiritual health care plans Providing regular updates to manager and team regarding clinical care Attending weekly team meetings Initiating contact with other clinical care team members as appropriate Providing referrals within Atlantic Health System (Behavioral Health, Social Work, Humanistic Medicine, support groups, etc.) when needed Providing referrals to community-based spiritual support resources, to include connecting patients to faith communities, caregiver support, and bereavement support. Contributing to the design and development of the Spiritually Integrated Network – the program database of Spiritual Care resources in Northern New Jersey Contributing to research components that could include data collection, data mastering, analytics, and quality assurance of outcomes Schedule is flexible and to be determined by manager and chaplain
Required:
Has an MDiv or MDiv equivalency with coursework in counseling Completion of minimum of four units Clinical Pastoral Education (CPE) accredited by the Association of Clinical Pastoral Education. Board certified or board certified eligible by APC, NAJC, NACC, or NAVAC
Preferred:
Candidates local to Northern New Jersey Has a commitment to meeting the unique spiritual care needs of patients and their loved ones from diverse spiritual, religious, and cultural backgrounds. Is committed to working collaboratively and creatively on an interdisciplinary team Has a demonstrated interest in Tele-chaplaincy, Outpatient Spiritual Care, and/or Population Health approaches to Spiritual Care Has basic research background or experience Is passionate about providing one-to-one Spiritual Counseling Has sufficient fluency with technology to work remotely at home Spanish speaking
Other Details
Weekly per diem hours vary depending on available caseload Patient care is virtual (video and telephone), but some in-person patient care possible if desired by patient and approved by manager.
