In this role, you will support your patients by providing direct care and coordinating with the interdisciplinary team to ensure that both patients and their families have an optimal care experience. Responsibilities Perform ongoing evaluation of patient’s functional statusTeach and/or retrain patient’s skills in daily activitiesProvide instruction to patient’s family/caregiver on treatment regimenFollow treatment program and goals for improved patient function as advised by the Registered Occupational TherapistMaintain appropriate clinical records and submit documentationEnsure compliance with all state and federal legal and regulatory requirementsAdditional responsibilities as assigned Qualifications Graduate of an occupational therapy assistant education program accredited by ACOTENBCOT certified with current licensure to practice in state of operation1+ year COTA experienceHome health experience preferredCommitment to clinical and documentation excellence About our Line of Business At At-Heart HomeCare & Hospice, our primary ambition is to bring first-class care to the warmest and most serene environment possible: home.
